How to Submit Your Forms:
Download patient forms from our website.
Complete them through MyChart or print and submit them via:

What to Bring to Your Appointment

Arriving for Your Appointment
Whether you're a first-time visitor or returning for a follow-up, here's what you need to know about arriving for your appointment.
First-Time Visitors:
Arrive 30 minutes early to complete registration and paperwork.
Use MyChart ECheck-in up to 7 days before your visit to update forms and insurance details, and pay your copay.
Follow-Up Visits:
Arrive a few minutes before your scheduled appointment time to keep things running smoothly.
Parking Information:
Parking is free for the first hour, and $4 for anything over 1 hour.
Payments are accepted via cash or card (no American Express).
Rescheduling and Cancellation Policy
We understand that sometimes appointments need to be rescheduled or canceled. To avoid a $50 fee, please notify us at least 24 hours in advance.
No-Show / Same-Day Cancellation Fee:
$50 fee applies for missed appointments or same-day cancellations, effective since November 2022.
Results and Disability Forms
Here’s what you need to know about receiving your test results and completing disability forms.
Test Results:
Most results are available within 7-14 business days.
For imaging done outside Piedmont, please ensure the results are delivered to our office.
Disability Forms:
Allow 10-14 days for completion.
Let us know if you would prefer to pick up the form or have it faxed or emailed.
